Please note this is a hybrid event - you have the option to attend in person or virtually. Registrations are required for all attendees. For in-person attendees, the event includes a one-course lunch with a glass of wine and self-service coffee/tea & cookies. Virtual attendees will be sent a link for the event via email. This event is co-hosted by DACOR and the Senior Living Foundation.

April 16th is National Healthcare Decisions Day! Join us to complete or update your End-of-Life Healthcare documents (there are more than you might think!) and become familiar with all the options one has available to control and orchestrate the experience you want during your final months, weeks and days.

All participants will come away with completed documents and conversational tools that will help you inform your friends, family and doctors what your wishes are.  
 
All these components, working together – will arm you and your trusted, confident and tenacious Medical Power of Attorney advocate, is the only insurance you can have for having an end-of-life experience that comports to your own personal values.  
 
To ensure you have the experience you want at life’s end is by getting crystal clear with and putting down in writing your End-of-Life Wishes, your personal values and knowing your options so that you can communicate all of that before you are in crisis with your loved ones and friends – particularly the person(s) you designate as your Medical Power of Attorney for Healthcare Decisions. 

In this Hybrid Webinar held on National Health Care Decisions Day - April 16th from 10:30 – 1:30 at the DACOR Bacon House and online, DACOR members and Senior Living Foundation donors will be provided with all the official forms specific to your state and template documents you will need to ensure your end of life wishes are respected and carried out - especially if you are not able to speak for yourself when the time comes. 
 
In person workshop attendees' ($30 registration charge) includes a one-course lunch with a glass of wine and a self-service coffee/tea & cookie station, along with a hard copy document packet.  
 
Hybrid attendees (no fee) will receive links to digital documents to download and print at homes to be ready to fill out during the webinar.  

All attendees are encouraged to consider a free-will donation commensurate to the value they place on the information received to the DACOR Bacon House Foundation House Preservation Fund and / or to join DACOR if you aren’t already a member (which is gratis for those 90+) All DACOR memberships come with legacy membership eligibility for children and grandchildren regardless of their careers which is another meaningful legacy to leave.

Jared Hughes is Director of Development of DACOR Bacon House Foundation.  Before coming to DACOR in June of 2021, Jared worked for eight years at Compassion & Choices – the nation’s oldest and largest end-of-life options advocacy organization – where he advanced millions of people’s right to control their dying process and become educated about their end-of-life options to avoid needless pain and suffering. For two years he advanced the mission of nation’s oldest and largest continuously operating non-profit hospice (Capital Caring) here in the DC area.  He commendably served as a medical power of attorney for both his late parents and attended their peaceful and deaths having advocated for their end of life wishes. He has read extensively on the subject and most recently attended the University of Vermont’s Death Doula Training. He envisions a society that is much less death-phobic, where everyone has access to the tools to have agency over their final days, free from pain, so everyone can focus on orchestrating a beautiful and meaningful transition to the great beyond that doesn’t leave loved ones traumatized and bereft.  He looks forward to sharing decades of knowledge and advocacy experience with you and being of service in this way to his Foreign Affairs Family. He is available to discuss your specific questions and concerns as a follow-up to this session.
